All clinical assessment and professional documentation services are private pay. Insurance is not accepted for these services. Fees vary depending on the type and complexity of the assessment or documentation requested. A consultation is required before scheduling so that we can determine whether the requested service is clinically appropriate and within the scope of services provided by Phoenix Rising.
